Abstract:

This one-page handout is an easy-to-use self-management plan for people with congestive heart failure. It uses flags (green to show good health, yellow to give warnings, and red to indicate health emergencies) to help patients identify symptoms and understand what they mean, seek assistance before a crisis develops, and take appropriate action in the event of an emergency. It contains a checklist of things people can do every day to keep their symptoms under control, and includes blank spaces to write down instructions and contact information for their health care provider. The English version contains an additional fact sheet about congestive heart failure that is not included with translations of this self-management plan.
