Heart Disease Self-Management Plan
This one-page handout is an easy-to-use self-management plan for people with heart disease. It uses flags (green to show good health, yellow to give warnings, and red to indicate health emergencies) to help patients identify symptoms and understand what they mean, seek assistance before a crisis develops, and take appropriate action in the event of an emergency. It contains a checklist of things people can do every day to keep their symptoms under control, and includes a blank space to write down the blood pressure reading they should stay under, and instructions and contact information for their health care provider. The English version contains an additional fact sheet about heart disease that is not included with translations of this self-management plan.
