Abstract:

This two-page clinical tool gives healthcare providers a set of predetermined, standardized questions they can ask patients to identify common patient care issues, offer pertinent guidance and information materials, and suggest preventative measures and other courses of action for effective health management. The document is presented in a checklist format, with a space for providers to indicate where topics have been discussed and materials given. It asks key questions about healthy lifestyle, including exercise, diet, weight, and tobacco use. It advises prevention through regular medical check-ups, and outlines medical appointments, medical records, and Medi-Cal renewal. Screening questions for domestic violence are included, and detailed prevention education questions about Tuberculosis (TB) disease and Latent TB infection (LTBI) are provided.
