This six-panel brochure educates people with diabetes about the importance of testing their blood glucose level and keeping it in control. It describes the HbA1C or A1C and finger-stick tests to check blood sugar. The document provides a checklist of what to ask a health care provider about testing, and a space for patients to enter their ideal finger-stick test results before and after meals and at bedtime.
