This four-page illustrated handout is a communication tool that health care providers can give to patients to help them express pain level, health conditions, and requests to staff. It combines the 0 to 10 scale with the Wong-Baker FACES Pain Rating Scale to help patients with limited English proficiency express their level of pain. It contains illustrations with corresponding descriptions of health conditions such as chest pain or nausea, and includes illustrations communicating requests for food and water, bathroom, and other needs. The document uses images and simple text to identify objects and persons, mark time, and make basic requests.
